Maybe 'Revoked my Title'-opinion modifier should block them from being granted titles?
I'm having a similar bug, but I think I can see why it's happening:
Somehow, the city of constantinople became the county capital, so the new empress keeps revoking the title from the burgher in charge, then returning it to him. I'm going to guess that she would revoke a barony as well, but the current council wouldn't allow her to.
It's annoying for me, since I (King of Trebizond under the empress) am being warned about it every 10 seconds or so.
And since she's 2 years old, this is going to continue for a while.
