Finally, although not as obvious a change as those listed above, I'd use nordic instead of scandinavian. :)

These are not bugs and I believe the middle two have been discussed before and we decided to stick with what we have.

That would be incorrect.
Nordic countries are Denmark, Finland, Iceland, Norway and Sweden, and all islands between Denmark-Norway and (including) Greenland. Scandinavia is Denmark, Norway and Sweden. Scandinavian languages are the Northern Germanic languages spoken in Denmark, Norway and Sweden. Finnish belongs to an entirely different family, as do the languages of the indigenous populations.

Oxford English Dictionary: Nordic = native of Scandinavia, Finland, or Iceland.

Not sure if this a bug. If England gets cores on Holland and Artois through the
Jacqueline of hainault events, and manages to keep them they will not get any dutch revolt events (unless they hold a non core dutch province). This can also screw up the formation of the dutch republic events if they revolt from another country (i.e. any events with ownership of Holland as a trigger).

I've experimented with some more games since posting this and it seems that both England and Kleves are totally immune from the dutch revolts if they
hold cores in the Netherlands. This is regardless of religion and also if they own non core territory in addition to their cores.
On a side note another anomaly is that an innovative catholic will not get any revolts whilst an innovative protestant will.
