1. Galicia polaca
As Cataluna own and control Galicia and Volhynia.

First of all - two Galicias, and those achievements are quite fun, like with Georgias or Albanias-Iberias. Second of all Catalans are sometimes referred to as los polacos, which obviously also means the Polish.
TBF, I'm not 100% sure of referring to Catalans as Polacos isn't derogatory.

2. The End of Land
As Britanny own and control Finistere, Cornwall and Galicia.

All of the above have a peninsula/cape actually called "The Land's End".

The issue with the first one is that it involves releasing a country so I doesn't really work as an achievement/on ironman
Actually it does. They have to specify in every single achievement that this is not possible, so it should be no work to create one where it is possible!
 
I like the land's end being cornwall, brittony and galicia also based on the historic spread of brythonic languages after the anglo-saxon invasion.
