Guards keep opening the doors to solitary and lockdown prisoners

Since the patch, I keep seeing prisoners walking around my prison in cuffs. When I mouse over them, they still have a period of lockdown or solitary displayed and the timer isn't moving. I click on them and back to their cell/solitary to have a guard manually escort them back, which they do. Except the prisoners walk right out and the guards don't seem to care. Their timers aren't going down as a result and they are stuck wandering my prison in cuffs.

To see what's going on, next time I got the alert someone is awaiting solitary confinement, I followed him till he was placed in solitary and the solitary door closed. Guard walks away and everything looks fine. Except after a few moments (30 mins of the prisoner's 2h solitary stay has expired), a guard walks up and opens his door for no reason. Prisoner walks out in cuffs, his timer now stuck at 1h28m Solitary and he starts wandering the prison. There is nothing I can do, other than just end his punishment so he can go about his daily routine. Please fix this!

Edit: Added a screenshot below as an example. This guy is supposed to have a 3 hour solitary stay, guard takes him to solitary and leaves, prisoner just walks out and is now wandering in my yard in cuffs.

I'm having the same issue as well
 
Same problem here still and with the lockdown. It's so annoying and borderline game-breaking. A temporary fix I found is to just manually escort them back in and lock the door shut until their punishment is up. Although doing this for each and every prisoner is tedious and makes the game super boring for me.

Edit: I found out that they walk out because they are still scheduled to reform programs so they go to that room and come back into solitary/lockdown. But since they are in the punishment they don't even "attend" the class so they just walk around the room and come back when the class finishes.

Happening to me too and it's very annoying. Please fix it. It's happening in my prison without any work or education scheduled, they just walk out to the yard or wherever in free time. Wouldn't mind if I could schedule solitary-bound inmates seperately from other prisoners, so could have say 23 hours of lockdown, but that's not an option.
