Excuse me if this is obvious, but how does one annex nations in war? I haven't seen anything related to it yet. Is it possible, or do I need to vassalize first, which will be a pain because it is post-reformation Europe, and my vassals keep converting before I can annex.

If you want to annex an enemy during a war there are two ways: If the enemy is a pagan state you can simply annex it when you have a 100% warscore.
If the enemy is not pagan then you can only annex states that have only 1 province left. So for larger states you might need more than 1 war, first taking several provinces and annexing only in the last war when the enemy is reduced to his capital.

You´re welcome.
However remember that force-annexing states of your own religion is BAD and gives lots of badboypoints. Better force-vassalize and diploannex co-religionists.
