Well, I can't really find much on Manichaeans that could be translated into game mechanics. Neomanichaeans do seem to exist, though.

It obviously shouldn't be a heresy of Zoroastrianism, although it could possibly be part of the Mazdan group (or alternatively the Christian one, or even it's own). Manichaeans could possibly have access to absolute cognatic and female councillors and all that, like Messalians and Cathars, but unfortunately the only place I've seen gender and racial equality cited is on a neomanichaean's blog.

As for their replacement as a heresy of Zoroastrianism, I'd propose Zurvanism (which in game terms would be wholly indistinguishable from mainstream Zoroastrianism, as far as I can tell). Speaking of heresies, I don't think Manichaeanism would have any.
 
Manichaeism fits best as a new religion in the Mazdan group, and so does Yazidi for that matter. For unique decisions/events they can have the Bema as an equivalent to Ramadan or the Pagan festival events. The Christian monastic system can also fit, as the Manichaean clergy was essentially made up entirely of the monastic 'Elect.'

Mani did preach racial equality, but almost all of the monotheistic religions in the game (at least on paper) did as well so I don't think that would have any in-game effects. As for gender equality, women could become part of the Elect and were considered equals in that regard. However, this didn't translate over into the secular world, and the treatment of women depended on the local culture far more than the Manichaean faith.

The most obvious example of a heresy was a schismatic movement in Central Asia that broke off from the mainstream Manichaean hierarchy in Csteiphon, but their name escapes me. If another one is needed, then Mandaeism could loosely fit the bill, but they're more a related faith to Manichaeism than an offshoot or re-imagining of it.
