Can anyone point me to information about leader death in FtG?
I did not find anything except from EU2 that leaders may die in battles.

What I would like to know is what the percentage is for a leader to die before his deathdate and
if that risk is only checked while the leader is in a battle, in a siege or the whole time?
 
There isn't such a mechanism. Since you can't recruit new leaders, it would be a bit punishing to have your leader die prematurely. The only way to kill a leader is to annihilate his army or fleet.

annihilation is the only way a leader dies, .....since the default for Europe is to have annihilation of armies and navies as ON, then deselect this option as per the rest of the world.
I cannot remember where this file is for prevent annihilations of armies
