How do you decide who to appoint as a bishop's successor?

I have a few catholic religious holdings under my control, and it seems now I can appoint the next bishop directly. Do you appoint sons who have taken the vows? Is there any opinion bonus for appointing someone? Or is this all for the sake of spreading a heresy when that time comes around?

If your son has taken the vows, there is no immediate need to appoint them any ways, as they are already removed from succession. That being said, if you want to appoint them and support them for cardinal/pope, then that's one reason. Or just to give your family more holdings. But you can nominate any unmarried courtier. Of course, getting someone young who likes/is similar to you will help ensure that they pay you and not the pope.

All told I don't tend to bother with it too much, since it will work fine on its own.

Would you ever appoint your monk brother to be a cardinal? I believe vow-taking brothers still dislike each other. Any advantage to keeping it in the family?
