Wondering where I can find out the required events/actions needed for the "Triggered Only" nicknames in the txt file?

Some are easy, like 'the Great', 'the Holy', and 'the Conquerer'. Others are a complete mystery.
 
Is there a list of required conditions?
You want to look in the 00_nicknames.txt file which lists them all along with the requirements and chance of gaining it. If you run the game through steam then this will be in:

C:\Program Files (x86)\Steam\SteamApps\common\crusader kings ii\common\nicknames

What I am looking for, are what trigger the TRIGGERED ONLY nicknames in that file.

I believe these are created to give names to historical characters. They do not fire in the game.
 
You can actually get The Conqueror if you Request Invasion from the Pope and successfully get it. The Great can be given from the ambition Become Exalted Among Men, or by having a load of Prestige and no other real qualities. The Holy, IIRC, can be given by the Pious version of that ambition, the name escapes me at this time, but I never seem to be able to get either or those ambitions late game. :/
 
Searching these names briefly in the CK2 folder has revealed the following

it's Paragon of Virtue, and you have those (PoV and Exalted..) only available when your current Prestige/Piety is far enough from what is required... which often is not the case for heirs after a certain time ;)
