For me the main difference is that MEIOU just feels unfinished whereas D&T doesn't. MEIOU is also a fair bit different to the vanilla whilst D&T is more of a tweaking to it. The main problems I had with MEIOU were/are:
A)It's overly complicated. The advisers in particular are absolutely ridiculous. Every time you go looking for one you spend half an hour working out which one you actually need. Also, the map is, in my opinion, kinda ugly. It is an interesting idea to have the free imperial cities and (for lack of a better word) dynamic borders, but I just couldn't get used to it. Not sure if either of these just stemmed from lack of playing it and being used to the normal map though..
B)It has a lot of random events/decisions that are either meaningless or left in for unknown reasons. Quickly becomes irritating when you click on one by accident, inherit japan, and then have to clear your cache just to stop your european culture becoming japaneese with japanese troops...
C)It has crazy slowdown. My computer is built to run high-powered programs (such as video editing software) but I was unable to run MEIOU on the highest speed setting without suffering horrible FPS drops. This was probably my biggest problem with the mod as I like to be able to skip forward in times of inactivity (which seem to happen slightly more in the mod, especially within the HRE). For reference, I read somewhere that this might be due to the AI mod bundled with it, and I don't know whether disabling it makes higher speeds possible.
Don't get me wrong, I love some of the ideas behind the mod and the changes to cores, the HRE, religion etc, but the problems I had were just enough to put me off.
D&T on the other hand is just an improved version of vanilla with a lot of subtle changes that help to rebalance the game and make it run a lot better. It also looks stunning in comparison to vanilla; love the colour changes to the map and the changes to the coat of arms (in particular the fugly khanates such as nogai and the golden horde).
Anyhow, if you can put up with large slowdown on the high speed settings, and are prepared to invest some time learning how MEIOU works, it is definitely worth trying out. Changes the dynamics of the game somewhat, but almost all of them are for the better. It also gives a deeper overall experience than vanilla does. If you can't put up with those, or are looking for something simpler, then D&T is definitely worth a try. It's easily better than vanilla, but you still feel as though you are playing the same game.
Got a bit carried away with that, but just my personal experiences. I'm sure others have had better or worse experiences with either.

I like both, they each have their own flair.
MEIOU feels like a completely different game. It is by far the best looking mod out there, the map is perfect. The only thing preventing me from playing it over D&T is that it runs a bit slower.
