Yes, give to the heir before you die or watch for primogeniture.

Most counties, duchies and Kingdoms start with gravekind succesion law which split all your land between your kids. So it when says losses when you die it means most time that your kids will get some of your lands.

However, keep an eye on losses of inheritance in forigen land. Like if you married a ruler of some other land and you dont have successors from that section and you die. Someone could inheirt it from that section.

Find your heir. Then go to the diplomacy menu - it's under "Grant Landed Title".

Go to the diplomacy panel for your heir and select 'Grant landed title', then give him the title in question and you're sorted.

Edit: Just pipped to the post

I had this problem playing as Flanders. I switched to senriority so that I wouldn't keep losing my provinces, but there's a big downside: your rulers are almost always 40+ years old, leading to short reigns. On the plus side, switching to seniority meant elderly members of my dynasty in the HRE (Hainaut and Zeeland) inherited the duchy and so I added their land to my own. If you stick with gavelkind you have to juggle provinces between heirs just right so you keep as much as possible on succession, but that means having less land right now (and whenever you ruler looks like he's going to die soon).
