It's not. It's 238%. The 0.5 term is completely separated from the Tariff Efficiency; the overall efficiency is 0.5 * Tariff Efficiency, not 0.5 + Tariff Efficiency.
If you dial down the Tariff Efficiency to 200% in that example, you'll come up with a tariff of 13.23 and an income of 13.90. If you dial down Tangier's population units to 1.0, you get an income of 13.56. Which seems close enough since both of those provinces have buildings in them.
