I'll preface this by saying I have read both the manual and the strategy guide, and they give general answers, but nothing exact.

So my question is, how do you gain/lose points on these scores?

For instance, I know that turning a colony into a state nets you prestige, but how much? Is it always the same?

Is there a list of what gives which score? I know some of the exact triggers, but not all of them by any means. Is there a list anyone knows of?

I can help you for industrial score : it's the exact sum of the value of the manufactured goods you produce in your factories.

Military score : if I'm not mistaken, it's +2 per general and +0.5 per brigade you can recruit (not only the already recruited brigades). Also, big ships bring lots of military points.

Yes, Dreadnoughts give 20 points each to military. It's a very useful tool to keep ahead of the AI.

The total value? Or the number of goods total? See, I have a factory making 3,200 a day making cars, but my Industrial score is only 2,000 or so. I mean, that still makes me 4th, but yeah.

It's perhaps not the total value but it's directly linked to the price of produced goods. The test has been made by SashaM in the VRRP thread. So now in the latest beta of VRRP all base prices have been halved ; the balance between goods is kept but industrial power is now exactly halved. It makes it less important in the total score.
