I believe forming Prussia changes Prussian to your primary culture. Thus it is no longer an accepted culture.

Might be true, but I got that popup maybe 50 years after forming Prussia, and my culture has been Prussian since I formed it.

As far as I know, the papal state always survives one way or another even if it's annexed. I'm not sure if it's a revolt that happens, but it always pops up somewhere.
Heck, I've seen it in Germany, France, even in Newfoundland.
