Happens all the time. Basically, when you control an area on a different continent which is mostly covered by one nation's cores, rather than <Your Nation> <Continent> it'll be called <Your Nation> <Country>. In this case, Portugal no doubt has cores on Tripoli, whereas Tripoli itself probably only has cores on some of those provinces.
 
This even happens with formable nations.

Nah, unless you count the northern half of Germany and the Lowlands as a Colony. This game was just a quick "lets see how crazy I can go within 200 Years"-game, I didn't do anything except achieve hegemony in Europe and form and subsequently unite hindustan.
